這一份表是由美國籍方來做填寫的

★重要更新: 自 2020/06/23 起，USCIS  要求所有的申請人在填寫任何移民局的表格時，除非表格上有註明 '' Optional '' 或 '' If Applicable/Any '' ，不然所有的空格都必須填寫且禁止留空，如該項目對申請人不適用時請一律填入 '' None '' 或 '' N/A ''

★重要更新: 自 2020/09/05 起，所有 USCIS 表格如需要手寫的部分 (例如; 簽名或需靠手寫填上者)，一律只能使用黑色原子筆(black ink)，未來使用藍色原子筆(blue ink)或鉛筆者會直接退件

1a. Family Name(Last Name): 填入台灣籍方的姓氏

1b. Given Name(First Name): 填入台灣籍方的名

1c. Middle Name: 填入台灣籍方的中間名

Mailing Address

2a. In Care Of Name: 填入收件代表人的名字，這格通常填入的是幫你們代收信件的人的名字

2b. Street Number and Name: 填入街道名稱

2c. Apt.=Apartment    Ste.=Suite    Flr.=Floor

2d. City or Town: 填入該地址所在的城市或鎮

2e. State: 填入該地址所在的州

2f. Zip Code: 填入該地址的郵遞區號

2g.-2h.: 新版表格的設定是如果地址是在美國境內那這裡就會鎖起來，所以直接留空跳過即可

2i. Country: 填入該地址所在的國家

Other Information

3. Country of Citizenship or Nationality: 填入台灣籍方的國籍，記得寫 Taiwan 就好了，你寫 ROC 的話那你拿到的綠卡上就會被註記為 '' China, People's Republic of ''

4. Date of Birth: 填入台灣籍方生日

5. Alien Registration Number(A-Number): 填入台灣籍方的外國人號碼(A開頭)，有些海關會在你持K-1簽證入境時就核發這組編號給你

6. USCIS Online Account Number: 台灣籍方如果有 USCIS 官網帳號者才要填

7. Daytime Telephone Number: 填入台灣籍方的日間連絡電話

【 手把手填表教學-Part 3 】: 以下是單純只有美國籍方擔保美國籍方之外國人配偶的情況   (household size: 2)，如果美國籍方在婚後有領養外國籍配偶與前段婚姻或他人所生的子女者，則以下不完全適用
